Payment Policy
Payment is due in full on the date services are rendered.
We do not offer in-house payment plans. However, for your convenience, we accept the following forms of payment:
- CareCredit: Apply online (www.carecredit.com) or in our office.
- Credit cards (3.5% Fee Attached to any and all credit card payments)
- Debit cards
- HSA/FSA
- Personal checks
- Cash
Any portion not covered by insurance, including deductibles, co-insurance, co-payments, and non-covered services, is the responsibility of the patient and must be paid at the time of service.
Outstanding Balances
Any outstanding balances must be resolved prior to scheduling future appointments. Accounts not paid in accordance with this policy may be subject to additional collection procedures.
